Exjade kidney problems leads to FDA warning

The FDA and drug maker Novartis AG have issued an important warning about problems with Exjade which could lead to kidney failure or death. Information about Exjade side effects were outlined in a May 14, 2007 letter to healthcare providers which was posted on the FDA website today.

Exjade, manufactured by Novartis AG, is a drug that is given to patients undergoing regular blood transfusions to remove excess iron. It was approved in November 2005, and over 13,000 patients have received the drug.
Reports found cases of acute renal failure among individuals taking Exjade, and there have been at least 8 reported deaths. The actual number of deaths caused by the drug could be significantly higher, as experts have previously estimated that the number of adverse side effects which are reported is often as low as 1% to 10% of all problems encountered.
Blood transfusions (transfusional ehmosiderosis) result in patients receiving red blood cells that contain iron. In some patients, the iron can build up and lead to damage to the heart, liver or other organs if it is not properly treated. Exjade is prescribed to remove excess iron, and it is approved for use in adults and children over the age of two.
The drug’s warning label has been updated to include information regarding Exjade problems with kidney failure, as well as indication of reported problems involving blood deficiencies, known as cytopenias, which can be fatal, blood vessel inflammation, hives and hypersensitivity.
Concerned patients should contact their physician. Doctors were cautioned to monitor serum creatinine for those at an increased risk of complications, including those with pre-existing kidney problems, older patients, and those who are using medical products which reduce renal (kidney) function.

هل مرضى الثلاسميا سريعوا الإجهاد؟
وفقا لما يقوله الأطباء انه إذا كان هيموجلوبين مريض الثلاسميا فوق 8 جرام فإنه يمكنه
أن يمارس حياته بشكل طبيعى , وستدهشون حين أقول لكم ان مريض الثلاسميا يمكنه
أن يمارس الجرى والسباحة ويلعب الألعاب العنيفة , ويذاكر وينجح بتفوق وحتى إذا كان هيموجلوبينه8 جرام لا أكثر,
إن الحركة والنشاط مفيدة جدا لمرضى الثلاسميا.
لكن إذا كان الهيموجلوبين منخفض إلى 5 جرام فيجب على المريض أن يبادر إلى
نقل الدم , وسوف نتحدث عن مناهج نقل الدم فى بند مستقل إن شاء الله.
إلى أى مدى يمكن لمريض الثلاسميا أن ينخفض هيموجلوبينه ؟
إذا امتنع مريض الثلاسميا (الحالة الكبرى) عن نقل الدم مدة أربعين يوم وكان
هيموجلوبينه السابق 7 جرام , فإنه سيصل إلى 4.5 جرام تقريبا حسب عدة عوامل
وقد يصل إلى 3.5 جرام , وباستمرار الإمتناع عن نقل الدم يستمر الإنخفاض فيصل
إلى 3جرام ثم 2جرام إذا بقى حيا وفى هذه الحالة قد لا يستطيع مريض الثلاسميا البقاء حيا
لا ينبغى أبدا أن يترك مريض الثلاسميا حتى يصل إلى هذا الوضع المتدنى
وعلى كل فمريض الثلاسميا لديه قدرة ما على تحمل الإنخفاض
فى الهيموجلوبين إلا أن أعراض الأنيميا الحادة ستظهر عليه بشدة وتتأثر جميع
أجهزته ووظائفه الحيوية تأثرا شديدا , نذكرمنها :
1- ازدياد كمية الدم مع بقاء الهيموجلوبين منخفضا(يشبه هذا تحول الدم إلىماء/بلازما)
2- وبسبب ازدياد كمية الدم مع انخفاض الهيموجلوبين يتعب القلب بشدة وربما يتضخم.
3- يتمددالنخاع ويتضاعف داخل العظام فتصاب بالهشاشة وتصبح شديدة الضعف.
لأن النخاع يهدف من ذلك إلى زيادة الهيموجلوبين لكنه يفشل باستمرار فيزداد تمددا
وعملا وإجهادا بدون فائدة .
4- يزداد تضخم الطحال وفى حالة كونه غير موجود(مستأصل) تقوم الكبد بتكسير
كرات الدم الحمراء.
ما هو الهيموجلوبين المثالى لمريض الثلاسميا ؟
بالنسبة لمريض الثلاسميا البالغ إذا وصل هيموجلوبينه إلى 9 جرام (نعم 9 جرام) فإن الأطباء فى مصر
ينصحونه ألا يخضع لنقل الدم وأن يجرى تحليلا للهيموجلوبين بعد اسبوع , فإذا وجد أنه
لم ينخفض عن 9 جرام يستمر الوضع هكذا يجرى تحليلا بعد اسبوع وهكذا
, إذا فالأطباء يقولون انه إذا كان هيموجلوبين مريض الثلاسميا
9 جرام فلن يستطيع أن يرفعه أزيد عن طريق نقل الدم إلا إذا خدع
الأطباء وقدم لهم تحليلا يثبت ان هيموجلوبينه الأن أقل من 9 جرام أو عليه أن
يجد طبيبا يوافق على إجراء نقل الدم مع العلم ان الهيموجلوبين لايقل عن 9 جرام
, ويحدث أن يأتى بعض الآباء بصحبة أبنائهم مرضى الثلاسيميا ويصر الأب على
ضرورة نقل دم لإبنه لأن لديه امتحانات أو مصيف على الرغم من ان الهيموجلوبين
لا يقل عن 9 جرام و قد يسمح الأطباء بذلك .

ان مسألة جعل هيموجلوبين مريض الثلاسميا مرتفع دائما مسألة هامة جدا ومفيدة
لمريض الثلاسميا وسنناقشها فى بند(مناهج نقل الدم) إن شاء الله .

ترسب الحديد

توضح الصورة (النموذج) الغدد والأعضاء التى يترسب فيها الحديد فى حالة زيادته لدى مريض
الثلاسيميا إذا أهمل فى العلاج بمستخلصات الحديد
1 - الغدة النخامية 2- الغدة الدرقية 3 - القلب 4- الكبد 5- البنكرياس
6- الغدد الجنسية ( الخصيتين عند الذكر _ المبيضين عند الأنثى)
